Healthcare Platform
This initiative aims to establish a platform that seamlessly connects at-risk patients to the care they require. Central to this system is proactive outreach and care, along with empowering patients through personalized care plans, education about their conditions, and the support needed to navigate their journey toward condition management and recovery.

The Problem
Care for individuals at higher risk of serious health conditions can often feel impersonal and insufficiently supportive. In this comprehensive proof of concept, we investigated ways to enhance the experience for at-risk patients by ensuring they remain connected to their care through a single source of truth. Our approach includes education, monitoring, regular touch-points, and a personal care concierge to assist them in navigating the often complex journey of seeking help and maintaining their health.

The Users
Patients are individuals identified as being at high risk for serious health conditions due to their medical history. They require a more integrated healthcare approach that offers personalized support through a health concierge, along with access to tailored educational resources, mental health assistance, and a comprehensive health maintenance plan.
Providers are the professionals assisting these patients and need a strong tool to deliver an organized, personalized service. Each concierge must be able to manage multiple clients, stay organized with daily tasks, and have detailed information about each client to effectively address their unique needs.

The Solutions
The patient side was developed as a mobile app, enabling individuals with limited resources to maintain continuous access to their healthcare information. It included scheduled mental health surveys, access to relevant educational and support resources, and a centralized location for their health records.
On the provider side, the admin interface was designed as a comprehensive hub for concierges to offer full support to their clients, consolidating all essential information in one platform. This interface featured daily and upcoming task lists related to patient care, detailed patient information with health insights derived from medical records, recommended next steps, and tools for organizing and scheduling services on behalf of the patients.

The Impact
As this was a proof of concept, the real-world impact had not yet been assessed when I transitioned off the project. However, I can confirm that we conducted extensive testing and feedback sessions with potential patients and healthcare providers, which significantly influenced many features and design decisions. The feedback we received was overwhelmingly positive, and the healthcare community we collaborated with was very enthusiastic about the potential of these tools becoming a reality.
